I hereby certify, on honor, that I was Captain of Company M. of the 2d Regiment of Maine Cavalry Volunteers, and am well acquainted with Joshua R. ADAMS who was a member of my Company. When he entered the service of the United States he was sound in limb and health. He was disabled while in the service of the United States, and in the line of duty, at Marianna, West Florida on or about the twenty seventh day of September 1864, by reason of a gun shot wound in the left foot, entering between the 1st and 2d toe passing up the whole length of the foot and logding in the Bones of the instep near the ankle joint. Received while making a charge (with his Company) upon a force of Rebels and the Battle of Marianna Fla. My knowledge of the above facts is derived from the following source: From personal knowledge having been present at the time of the occurrence. And I further certify, that I have no interest in said Joshua R. ADAMS' claim to a Pension. [notarized signature] John H. ROBERTS (late) Captain of Co. M 2d Me Reg't. Cavy."
